The Platform

Routine primary care, transformed into health intelligence.

The D3C™ surveillance platform transforms routine primary care delivery into a continuous, community-level health intelligence system. Every patient encounter at a kiosk site generates structured clinical and SDOH data (HIV, cardiovascular disease, diabetes, mental health, nutrition), feeding national DHIS2 systems in real time through standards-based FHIR R4 data feeds.

Solar-powered, off-grid operation

72-hour autonomous operation without solar input. Photovoltaic array with lithium battery backup. Deploys in facilities with no grid electricity.

Offline-first FHIR R4 architecture

Encrypted local storage with selective sync. Data collection continues regardless of connectivity. One-way FHIR R4 feeds to national DHIS2 systems.

24-hour remote monitoring

Remote monitoring centre staffed 24 hours, 7 days per week. Automated device health alerts and remote troubleshooting. No on-site technical staff required.

Multi-disease simultaneous surveillance

HIV · HbA1c · Cardiovascular · PHQ-9 mental health · Nutrition · NTD surveillance, all from a single patient encounter.
